Seattle - Durability - Keeping water out.
We used moisture barrier under the sill plate (above, left), and one also under the footings (above, center). The diagram (right), shows the entire system.  The Delta-drain is the black and gray fabric in the far right photo.
We took extra precautions to keep ground water out of the house, both the increase durability, and to prevent any possible mold problems.  Our goal was to make sure there were no paths for water to either leak up, or move upward thru capillary action.  These extra precautions are easy to put in during construction, and hard to retrofit.  The additional cost is quite small.  We used the delta-drain fabric as an added barrier, because our site has a lot of groundwater during the winter.  The foundation is also covered with two coats of Thioroseal.

We put perimeter insulation under the slab because its cheap to do during construction and it makes it more reasonable in the future to retrofit part of the basement to be heated space.
